How can i obtain a birth certificate if was born at home

Hi, I was born in Madhya Pradesh and do not possess a birth certificate as i was born at home and my birth has not been registered with the Municipality. I only have my baptism certificate a my proof of birth. How can i obtain a birth certificate.

Option 1:- Special Executive Magistrate (Local Area Concerned) has powers to issue Birth Certificate in the format annexed below

Sub: - Request for Issue of the Birth Certificate of my son/daughter-regarding.
I, ______________________S/O / D/O / F/O / M/O / W/O________________________,
Age ____ years, occupation: _________, R/o H.No. ______, Near______________,
____________ Mandal, ________________Dist. Telangana submit the following few lines for
your kind perusal and sympathetic favorable consideration please.

That my wife’s name is ___________________ Age ________ years and my son/daughter
___________________was born on__________ (dd/mm/yyyy) in _______ Delivery at
H.No.________, Near________, ___________ Mandal/Municipality, _____________ Dist.
Andhra Pradesh. The information regarding birth of my son/daughter was not informed to the local

In the event of the Option 1 not feasible/pragmatic for any reasons whatsoever you have the option of preferring an Application before the Concerned Judicial Magistrate i.e. JMFC (Judicial Magistrate First Class)/Metropolitan Magistrate in case of Metropolitan Cities. The Application must contain the reasons for not registering and Urging the Judicial Magistrate for an order directing the concerned Authority (Local Gram Panchayat, Municipality, Municipal Corporations) to issue a birth certificate pursuant to the Judicial Order passed by the Magistrate.

Needless to say that the Competent Authority (issuing the certificate) should be made as Party Respondent and you being the Applicant.

You can get your Non Birth Registration Certificate from Municipalty. You are able to apply under birth and death registration act in the court. Where you have finished your education your need your all the certificate. You can get bonafide certificate (school) or leaving certificate. call for more information
