During marriage registration the husband's surname was added to a female candidate's name.. but she got divorced and continued with her maiden name.. Now during filling up the attestation form after being selected in a government job, it is asked to mention whether you have added or dropped any part of your surname or name.. Is it compulsory to mention the previous husband's surname, which was added in the marriage certificate.. She has her maiden name in all government records except the certificate of marriage.

She should attach all relevant documents including Marriage Certificate and Divorce Decree and also specify the same in her affidavit so later on no issues are created as it’s a legal requirement

Going by the facts provided by you, it is essential to keep the husband’s surname as they will written on the documents. It is better ti look at the government documents for the name before submitting the same.
