What is the process for obc certificate address change

Sir,I was a resident of a district from where my obc certificate was issued.But presently I'm a resident of another district.So if I need a new certificate prescribed by Govt of India format what would be address in the new certificate; previous adderess or present address as I have to submit income certificate from present address.please help.
As you are a resident in another district you should tag your original address as well as ask the local councilor to ascertain your stay in another district and thereafter get an affidavit from a first class magistrate too.
