My Light Bill & Photo Id have two different surnames. I want to submit a light bill for a Marriage Certificate. my actual surname correct on Photo Id. Will I have to give some affidavit on bond?

You can make an affidavit that both name belongs to you and your name is correct as per the photo id as in electricity bill your surname is wrongly mentioned.
What is written on photo id is your correct name and you can also get it notarized before notary.
