An application needs to be filed to the Issuing Authority i.e. Registrar of Births and Deaths under the concerned municipal corporation (SDMC/EDMC/NDMC) containing a brief statement and all the documents in favour of changing the date of birth. Hospital discharge certrificate of the birth is also needed if available.

If registrar rejects the application then you can approach the High Court under writ jurisdiction to issue order to the registrar to effect the change.

You have to make an affidavit mentioning the correct date of birth before the first class Metropolitan magistrate of your jurisdiction you have to also mention the wrong date of the date of birth.. then have to produce the affidavit before the concerned authority ..
