Fathers Name not present in School TC

I am a divorced parent with Visiting rights to my child - custody of child is with mother.However, it has come to my knowledge that my ex-spouse has deliberately left out my name in the Father's name field.
How can this be possible? What documents need to be produced for such an act? And I have not legally given up any rights on my child. And is this an act of forgery / crime?
Yes, it is a crime. She should have misrepresented before school. First get issue a legal notice to restore your name failing which ask them to furnish the documents on which they have accepted application without fathers name. In divorce decree there must be clear narration that you have give up all rights over child.
