Confusion regarding name change procedure

Hi, My wife did a name change in 2008 (not related to marriage, for personal reasons). The lawyer then told her to keep the newspaper cuttings of the name change ad. Later, when applying for passport, it was rejected because the passport officials said that the whole newspaper is required. Moreoever, this lawyer also didnt tell her about any Gazette publication if it is required or not. Many years later (now, 2018), we came to know that a Gazette notification is also required, which was never told to her by that lawyer. When we call that lawyer and ask him, he said that we have to shell 10k rupees to get that done! My questions: 1. We currently have the affidavit document on stamp paper + newspaper cuttings of the ad on the document. What else do we need? 2. Do we need to the Gazette publication? If yes, what is the process and the fees? Can it be done online?
