How to correct name in service book

Working in Central govt. PSU. I forgot to add initial at the time of applying for the job. Now I want to add the initial in service book. My initial is available in all other records like school certificates, adhar, etc. Pls let me know the procedure to add initial in service book too.
Consult an advocate and get one affidavit drafted by stating the correction required in your service book and get it notarized with you and your advocate signature and annex the supporting documents which you have stated above and give it to your authority for adding your initial to your name.
