Dear Querist,
You can first execute a Affidavit mentioning your maiden name and stating that you are changing your original name to the current name and get it Notarized by the Notary Public and thereafter you can issue a paper publication one in English language newspaper and the other in Vernacular newspaper stating that you have changed your name and u'll be reffered by the current name. Keep the Affidavit and new paper publication carefully.

You have to swear to an Affidavit with regard to the change of name before the Notary Public and publish the same in some prominent local dailies preferably in one English and one local kannada daily.

There are three mandatory things, which are needed to be done for authorizing the name change:

1. Affidavit Submission: An affidavit needs to be prepared for name change.
2. Ad Publication: An announcement should be published in the newspaper.
3. Gazette Notification: A notification should be published in the Gazette of India regarding the name change.

For preparing an affidavit, you should visit to local notary and the procedure will take a month or two. The expenses will be around Rs. 2,000/-.

You will have to procure a name change affidavit and then give paper publication in two news papers one national and one regional news paper this is the general procedure for a name change. It would be more helpful if you can tell the purpose for the name change, then i can tell you if there are any additions to the same.

You can change your name by following the procedure for changing your name officially - the process for the same has been described below:

For change of name one has to get an affidavit prepared stating reasons for change of name and also the new name. Then the same needs to be published in two newspapers having wide circulation in your area, preferably one in English and other is the local language.

After due publication the same has to be published in gazette for notification with appropriate forms, fees and documents for publication.

After publication in gazette the new name comes into force and request for change of name can be made in all appropriate offices.
