There are two issues from your assertion.
1. livelihood : You can file a maintenance case against your husband.
2. Restitution :- You can file a case for restitution of conjugal life.
The 2nd option seems to me better as within which you can ask for maintenance.
For both the cases you have to know the postal address of your husband. you can mention the address of the matrimonial house as 2nd address and current address as first.
Thanks

Sorry to say, I think your Physical condition is main problem causing which your husband wants to terminate this relationship, Though he openly not to say it to you. But as a Lawyer, and after read your story carefully I found it that, he is not interested upon you due to your illness. I think at first you can talk you frankly and directly with your husband whether she wants to dissolve this marriage tie ? if his answer is yes, then you think for it. If his answer is no, then you asked him for your Treatment. Legal action will not necessary before clarify it.
