Depending upon the medical conditions, the Honble court may grant exemption. you need to submit the relevant and promising medical documents and certificate along with your affidavit to the court. in other case scenario the apperance is mandatory or else the prosecution may seek issue of warrant against absence

If your lawyer is attending regularly then you can get an exemption from attending court hearings unless trial starts and the judge is insisting you to attend for your cross examination. Do you have a bail ? If there is one then you may relax some of its conditions on the basis of your medical conditions.

An accused is required to appear personally before the court only at specific stages as directed by law or the court; at other stages, exemption from personal appearance may be granted under Section 205 of the CrPC. If you want, I can also provide this in very simple English, exam-oriented format, or tailor it to a specific case (e.g., 498A, bail, quashing).

No, the accused does not need to appear in the court in the proceedings under section 498a, provided that their advocate is present and can file an exemption on the client's behalf citing the medical reasons or whatever the case maybe.
