Procedure to get name changed in birth certificate

My child born on 8may2010 name as per birth certificate is : sai Krishna Manohar. As per Aadhar :Krishna Manohar. Now I would like to change both as Manohar. I do not have any proof regarding this change but all my family members accept this change. Please suggest how to proceed further to make the change.
