Change name of son (add mother maiden surname)

I am divorced, my son is minor. I want to include my surname in my son's birth certificate. He was born in Mumbai, now I stay in Raipur. What is the process for changing the name. i want to do it, so that this is reflected in all future documents (birth certificate, aadhar card, passport, etc.)

If you have divorce decree (certificate) then you can change your son’s surname at aadhar sewa center .after that you can change his surname in all documents on basis of aadhar card .I think firstly ,you should change your surname then it will become easy to change son’s surname .one more think if you have divorce certificate so you can see on that divorce judgement that custody of child is mentioned or not .If custody is mentioned then you may show to government department to change surname of both of you
