How to file for an Amendment of Address Change

I submitted an application for address change along with a copy of the Aadhar card. But they court said to file for an amendment of address change. Can you help me with the format and procedure for the same.

To proceed, we need to file an amendment application stating our intention to change the address in the complaint, clearly mentioning the new address. there is no specific format, we can give simple application.
Once the court allows the application - meaning it approves the amendment then we must physically update the complaint with the new address. The amended copy must be sent to the Opposite Party.
