Procedure to get my 2.5 year old son's name changed

I wanted to change the name of my 2.5 year old son. I was told only an affidavit will suffice for kids upto 5 yrs of age? Is it true? What is the correct procedure?

No, there is no need for an affidavit for an 2.5 year child. The documents required are - affidavit of a guardian ( mother or father) duly attested by notary/ oath commissioner/ first class magistrate along with the details of the either parent and 2 passport size photographs of either parent or a child self attested, the form should be printed on a plain paper with a signature of 2 witnesses and also by the parent, request letter along with request fee, original newspaper in which advertisement for change of name is published, CD containing the print matter without witness portion in MS word and type old name in front of signature, copy of birth certificate. All these documents should be deposited with municipal corporation of the state or country in which you reside.
