Procedure to change the name into sale deed

I purchased a flat in Hooghly-Chinsurah municipal area. The sale deed was registered at ADSR Chinsurah on 9/8/16. Till date the property is not mutated. Now meanwhile I have changed my name. I wanted to know the detail procedure to record the change of my name in the original sale deed.
You will have to apply for correction of deed. The procedure is first make an affadavit and then on the basis of this affadavit tell the vendor to sign on correction deed before the registar. That is how it is done.
