How to take original birth certificate from MCD

I have a query regarding the birth certificate of my Daughter. My daughter was born on 24-05-2015 and received temporary certificate from hospital. At that time i am not able to take original certificate from MCD and now few months back when i went to MCD office for getting original certificate ,i came to know that there is no registration entry on temporary certificate which was received from hospital and now hospital said that they have missed out to add my daugher name online in MCD website and now they will not provide any registiration number .pls guide what should i do to get birth certificate
you have to apply for birth certificate in mcd office a person there is entitled to give birth certificates and who will recieve the application from you and will register the birth in birth register maintained by the mcd and also will give you the certificate after few days of filing the application

You are second next honest person in this country as i feel ,if not go to any block of up and pay few rupees and get it done , Still not happy don't mind i am ready to file a case free of cost as it is a matter of daughter so only the legal stationary expence you manage and get the case file against the hospital for missed out the name and now expost facto sanction is required is not taking.

You may file a case against the Hospital for their negligence and request the court to issue directions for issuing specific directions to the MCD to carry out appropriate arrangements and get the birth certificate issued in the name of your daughter.
