To correct a name on a death certificate in Delhi, you need to apply to the appropriate local body Municipal Corporation of Delhi (MCD), New Delhi Municipal Council (NDMC), or Delhi Cantonment Board, depending on where the death was registered. The process typically involves submitting:

  1. A written application addressed to the Registrar of Births and Deaths.

  2. The original death certificate or a certified copy.

  3. Supporting documents showing the correct name (like Aadhaar card, voter ID, or passport of the deceased).

  4. An affidavit notarized on stamp paper explaining the mistake and requesting the correction.

  5. ID proof of the applicant making the correction request.

  6. Correction fees, if applicable.

For recent registrations (within 21 days of death), the hospital may assist with corrections. For older cases, especially those over a year old, the process may require additional verification or even court direction. Once processed and approved, a corrected death certificate will be issued.

Just prepare a judicial affidavit, mention the facts paragraph-wise, and declare that the two different spellings refer to the same person. Submit this affidavit to the concerned authority that issues the death certificate. The authority will make the necessary correction and provide a new death certificate as per your requirement.

You can obtain it by way of getting it corrected from a first-class judicial magistrate citing the correct facts and going to the corporation thereafter and applying for proper name correction and getting it proper.

Your question appears to be incomplete, as you have not mentioned the place of your father’s death or the cremation site from where the certificate was issued. However, if your father passed away within the jurisdiction of the Kolkata Municipal Corporation or any other municipal area in West Bengal, you will need to submit an application to the Deputy Municipal Health Officer. This application should include the original certificate issued by them, along with a copy of the death certificate issued by the hospital or doctor.

If there is a spelling mismatch in the certificate, the authority will issue a corrected certificate upon payment of the applicable charges, as per their standard procedure.

In case the death occurred outside West Bengal, you will need to follow the respective state's rules. However, the documents required for the application will generally remain the same. Please take note of this.
