First submit a written application along with proof of relationship, ID proof, patient details, and if available, authorization or death certificate. Hospitals generally maintain medical records and are required to provide copies within a reasonable time subject to their internal rules. If the authorities refuse or delay unnecessarily, you may send a legal notice and can also approach the appropriate court or authority seeking direction for release of the records. Preserve all acknowledgements and correspondence for future legal action if required.”

file an RTI with the concerned information officer for retrieval of the treatment of father's papers from the base hospital. RTI would have to be answered within 30 days and in case you answer is not received the first appeal can be filed to the concerned authority should you have any queries please feel free to contact me

You can file RtI aur can make any application and submit the same in the department. Best option you just file application where you mention reason why you need the document and submit in department where record room.. they will definitely give some reply to you

You may seek certified copies of your father’s medical records from the concerned Base Hospital/AFRA Medical Record Office by submitting an application with proof of relationship, ID proof and relevant documents. If denied, legal remedies including RTI may be considered.
