Query about an affidavit regarding addition of a surname

Sir,
i have my name,
as "Anurag kumar" in my birth certificate, 10th +12th certificate and in degree certificates(upto my graduation) and
as "Anurag kumar sharma" in voter id, pan, aadhar and all other documents(after my graduation).
my questions are:
1. can a gazette notification solve surname discrepancy in my higher studies or in case of a govt job ?
2. should i make "one and same person" affidavit or a name change affidavit ?
3. a notary affidavit or first class judicial magistrate afgidavit ?
no need to do name change affidavit. just a one and same person affidavit is enough. just tell that anurag kumar and anurag kumar Sharma are one and same person. That's all.
better to notarised affidavit by magistrate. this is the safest practice. That's all.

In your higher study authority what they are asking for 1st class judicial magistrate or notary if they are not demanding ok if you want to keep the as Anurag Kumar Sharma then change name affidavit of 1st class magistrate will be best and publish your name in a popular any language paper after affidavit
