Can a passport be applied again with an amended name

I have traveled to 3 western countries as a minor. At that time, name in passport was: RSG where G is my father's name. My parents somehow afforded to produce a birth certificate from a city of permanent residence (other state) which is different from the city where I was born. (Reason: my name was wrong in orig. certificate which no one bothered at that time). In 2010, returning to birth city for my secondary education, I changed name in that state's gazette as RG. Then, I applied and got a new birth certificate as RG. Now, all my higher secondary, PAN, Aadhaar, driving licence and college certificates are as RG. My passport expired 5 years ago. Can I apply for new passport as RG and relevant documents?

The various passport services are:
Issue of Fresh Passport: You can apply for fresh passport if applying for the first time.
Re-issue of Passport: You can apply for re-issue of passport if you want another passport in lieu of an existing passport for any of the following reasons:
Change in existing personal particulars.
Validity expired within 3 years/ Due to expire.
Validity expired more than 3 years ago.
Exhaustion of pages.
Damaged passport.
Lost passport.
While applying under Fresh Issuance category - please make sure you never held a Passport of the applied category (i.e. Ordinary Passport, Diplomatic Passport or Official Passport) in the past.

For instance, if you ever held an Ordinary passport (Deep Blue colour) in the past but never held any Diplomatic (maroon colour) or Official (white colour) passport - you need to apply under Reissue category if applying for an Ordinary Passport while choose the Fresh category if applying for a Diplomatic or Official passport. Similarly, if you have/held a Diplomatic (or Official) Passport but do not have/held an Ordinary Passport - you need to apply for Ordinary Passport under Fresh category while choose the Reissue category if applying for Diplomatic (or Official) passport.

You can apply for passport. As per gazette you have changed your name. So please attest a certified copy of the State Gazette at the time of renewal and also file a affidavit stating in detail that you have changed the name.

U have to file a petition for registration of birth where u were born along with document u have travelled passport would help u out but u have to take ur original name and names which were mentioned were minor names which could be used when u get originally name so file necessary petition
