What are the documents required for marriage registration

I have been married for 9 yrs. applying for marriage registration now, earlier surname(Grover) now its (Sharma), do i need any address proof with the surname Grover. I have only 10th certificate with Grover surname? Dont hv ration card or old voter id. what should i do ?
For the purpose of marriage registration you can file an affidavit alongwith your application swearing that you belongs to the same address as mentioned in your application, if you don't have any valid address proof.
