Procedure to add last name in all documents

Name on my birth certificate, college documents and passport doesn't have surname but i want to add my surname in all the doc.. what is the legal procedure

you have to prepare a notarized affidavit and produce before the concerned authorities stating that the mistake are to be corrected and file applications to that effect.In case of passport there will be prescribed form which you can down load form the MEA site.You may also have to publish in the gazette and keep & produce the copy of publication for further references.

Hello,

You will have to first get your 10th mark sheet corrected as it happens to be the basis for all other documents. For getting this corrected you will have to approach your concerned BOARD office and after that the process will start which requires publication in newspaper etc. If you want I can connect you to the right person.
