Mistake regarding how a Deed was prepared

A refi was done on the home Mr & Mrs ‘Smith’ owned as husband & wife, joint tenants. The new loan application also states Mr & Mrs ‘Smith’, husband & wife, as joint tenants. However, the new Deed reads Mr & Mrs ‘Smith’, husband & wife. Mrs ‘Smith’ passed away 5 years ago. Mr ‘Smith’ is now trying to refi again, and this error is causing a big problem. What should he do? Thank you.

you have not disclosed your family history. First of all take smc from the concerned authority and then apply for regn. of title documents in your favor in case there is no other surviving LH. pay stamp duty and then you can have refi.
