She can transfer the power of attorney on your name or she can make a deed of gift in favour of you. Once you prepare the document of transfer you have to visit to area SDM office where the property is situated for its registration.

If there is only the Power of Attorney in the name of your grandmother, then I am sorry to say, she cannot transfer the title or either gift the said property to you.Because power of attorney did not confer any title over the property which means that she does not have the ownership right over the said property,
For making the gift deed in your favour 1st and foremost, your grandmother ought to have the sale deed in her favour and then only she can gift the said property in your favour.
