Motor accident FIR in case of fatal accident need to know procedure

few months ago i was throwing a beer bottle on side of road by mistake bottle colided with divider and broke.most probably glasses would have come on the road.Now i want to make sure no accidents especially bike accident was caused due to this.How do i make sure of it i am feeling guilty of my mistake I checked FIR record of station falling under that area No news on any accident & no FIR related to that My question is 1. Is it mandatory for police to file fir in fatal accident or death cases
