Your all documents required to be seen to arrive a final conclusion but apparently it seems a case of medical negligence and deriliction of duties. You can file a case in consumer court after serving a legal notice .

Dear Sir,
You can file a complaint before State Consumer Disputes Redressal Commission. The cases of medical negligence are very technical as there are limited principles. For better consultation and guidance I need to examine the complete in-patient record.
Kindly note: There is a limitation period of 2 years.
We can fix appointment.
Thanks
