Where to get self-declaration for the surviving member certificate? We are filling the form online and the wife requires self-declaration. where to acquire it?

You need to approach the SDM office of your local jurisdiction in order to apply for the surviving member certificate. The death certificate of the deceased, the identity proofs of the the legal heirs, their passport photographs are needed for the same. you may consult me regarding this.

You need to apply for surviving certificate either online or offline at SDM office of your jurisdiction! Online would be more convenient in this regard. Else u can also approach SDM office ! Thanks n b in touch

You need to file it an application with the SDM for surviving member certificate. edistrict delhi govt. I am unable to copy paste it here. And if you want to apply online then you have to go to a particular district website.
