Depression issue in husband and he refused treatment

Hello Team,
I am married for 8 years and My husband was physical abusive 3-4 time. Recently he threatened me with Knife. 3-4 years before marriage he was under treatment of depression for 2 months in Hospital. I do not have much information about it . He is also not ready to consult a doctor.
Can I create an affidavit /legal document and register it in court. I need to include his anger issue, relevant nature to consult doctor and life threating actions. If anything happens to him or Me he will be responsible.
Dear queriest, as per your query you have mentioned about the fact that your husband is suffering of some mental illness like depression etc. due to which his behavior is very much disturbing, for the same you have to take him to the hospital and start treatment on him. if not then you have to suffer the same. try to make this happen otherwise it may lead to some unwanted issue

In this case it is advisable that you should probably go for the divorce proceedings under the ground of metal and physical cruelty. You can also avail ground of unsoundness of your husband.
You can create an affidavit to that effect before the court. You can also avail protection orders from the court in case you feel unsecured to live with him
