death certificate name modification

My uncle's name mentioned on death certificatel is as SUBASH CHANDER ARORA .Issue is in some places his name is mentioned as only SUBASH CHANDER ..i.e. without surname and in some cases as SUBASH CHANDER ARORA with surname QUERRY- can i get his two death certificates one name as SUBASH CHANDER and otherone with name as SUBASH CHANDER ARORA proving both are one person OR. can we get 1death certificate mentioning both names in one SUBASH CHANDER alias SUBASH CHANDER ARORA

hello hello sar madam ismein bahut jyada problem ki baat nahin agar do jo hai aapka 10 certificate Nikal Gaya hai uncle ke aapki to ismein koi badi problem nahin hai usmein Jo unka Naam actual mein hai uska jo hai agar actual Jo name hai usi ko jo hai usko aage badhai iske liye agar aapko kahin per lagana hai just certificate uska kuchh bhi koi dikkat nahin hai Bus simple sa ek jo hai aap apne taraf se ya koi bhi jo ek affidavit Jahan per bhi lagana ek affidavit jo hai aap vahan per de dijiye koi aur jyada problem nahin hoga
