Procedure to change name of the daughters

I was trying to change my daughters name for which i submitted all documents also but thy r denining to change complete name plz let me knw how to do it
Sir, first let me know where you have submitted the documents and who are denying to change your duaghters name.
FOR CHANGING THE NAME first you have to go to local notary and make an affidavit regarding change of your name and then you have to publish it in the newspaper and then you have to get your name published in the official gazette of your state
so let me know whether you have followed this procedure or not
