Dear Querist,
You will have to prepare an Affidavit informing the maiden name and the reason for the proposed name change. It has to be notarized and paper publication may be given in vernacular language and english daily.

Simple affidavit is sufficient to change the name, but if it is in the birth certificate, you will have to file a suit for declaration and mandatory injunction against the state Govt., by following the procedure u/s 80 of CPC.

1) Submit an Affidavit to a First Class Magistrate/Notary Public/Oath Commissioner in the District court.
2) The next step is to publish an advertisement in two newspapers- one in the official vernacular language and the other in English.
3) The next step is to get your name published in the State/ national gazette.
4) For changes in passport, driving licence, etc. the procedure will be as per the rules of the specific authorities.

You can change u r name by giving a Affidavit and getting it notarized and giving paper publication but this will not help u change u r name in other doc.mentioned by u. But after getting notarized u can file a suit for declaration that u have changed u r name and get a direction from the court to the authorities to change u r name.

You can change your name by following the procedure for changing your name officially - the process for the same has been described below:

For change of name one has to get an affidavit prepared stating reasons for change of name and also the new name. Then the same needs to be published in two newspapers having wide circulation in your area, preferably one in English and other is the local language.

After due publication the same has to be published in gazette for notification with appropriate forms, fees and documents for publication.

After publication in gazette the new name comes into force and request for change of name can be made in all appropriate offices.
