Procedure to change last name in school passing Certificate

Sir, i have change my name through gazette becoz of this i have git new adharcard pancard n otherbank details.recently i have blessed with babt girl so i went to nmc to get birth certificate but nmc is asking for my school leaving but my problem is on my school leaving my old surname is written but i don't want to make birthcertificatw of my daughter for this so plz guide what to do . is there any solution for this.
Give your school leaving certificate as well as your gazette copy. Once your name is changed, no authority can use your old name for any more documentation. may be they are asking for your leaving certificate to verify your old name and you are the same person.
