How to fix my name and d.o.b in my Birth Certificate.
14-Nov-2023 (In Documentation Law)
My real and only name is Rakesh; and my real d.o.b is Sept. 1992 but when i got my records checked in my district civil hospital they issued me a birth certificate which displays my name as " Jai " and my d.o.b as Dec. 1991.
I don't know how to fix it now. Please help me. I need it to be fixed asap because i cann't apply for passport without it and my further education plan are getting delayed.

You must be having 10th grade (high school) mark sheet. You can make an application to the city board of your town along with your 10th mark sheet in the form of an affidavit which will make your name change in the records. You can also apply for domicile which can help you put your case forth.
