How to file claim against Unitech Builders?

As per the honorable Supreme Court of India's directive, I need to submit information in the "Claim Submission Portal for Unitech Buyers" for filing claim against Unitech builders. One of the document required is an attested affidavit (in RS 10 stamp paper) stating whether I want a refund or flat possession.Please me by providing sample affidavit in which I can fill up the required details

Dear Querist, the affidavit is required to be drafted after going through your documents pertaining to the purchase of the flat as per the agreement signed by you and builder. The affidavit contains your name, address with the statement referringvthe agreement and your consent for possession of flat or refund of money as per the direction of the hobble Supreme Court. So without going through the details of your payments made against the said flat allotted / not allotted and tuebsupreme court order in details, it won't be possible to draft a blanket affidavit. Unitech Buyers need to Register on www.amicusunitech.in to exercise their option of refund or possession .
Indian Buyers would be required to upload an Attested Affidavit of a Notary (on Rs 10 or more stamp paper) stating therein whether he wants “Refund” or “Possession”, also mentioning requisite details of Flat / Villa / Plot alloted, project name and amount paid.
It is advisable to consult a lawyer for the process of purchasing a Stamp Paper, submitting your details on the Affidavit and then attestation.
