How to change the name in birth certificate

I would like to change my daughter's name and give her initial/ surname of my name.. Is it possible.. Can a mother's name be added to a child's name as initial?
Yeah sure but u have issue notice to all concerned authorities and file Declaration case in court and by adding her surname bw can request to declare the name, next step s need to go with paper publication... If child is studying below 10th std by affidavit we can change it..

Hi, Yes you can change the name of your daughter as per your wish and choice and it is possible as per legal provision. You can also add the mothers name as initial to your child's name. All you need to do is to get an affidavit prepared and notarized. Submit the same to the department of publication for gazette notification once you have published the change of name in two daily news publications.
