how can i correct my name and date of birth in birth certificate?

My name is MOHIT GILL and date of birth is 24 dec. 1993. But in the municipal committee my name is mukhtyar singh and date of birth is 23 dec. 1993. I am doing b.tech . now in future i will need birth certificate for passport. Now i am so much confused. Please help me how the correction can be done in the birth certificate.
You will require a signature on the letter from the (Parshad) ur block representative and along with an affidavit from the court you would submit the same under a respective section applicable under a law in ur state to the municipal committee.
As you have not disclosed the name of the city as well as the state, i will not be in a position to tel you the applicability of law.
