My birth certificate contain only my first name, while all my other important documents (adhar card, PAN card, 10th marksheet) contain my full name with surname. If you require to make correction in your birth certificate, it will require providing a few important documents and giving the reason behind the change.
This will often require some form of proof in order to validate the change also have to obtain an affidavit from the court and submit the same under a respective section applicable under a law in your state to the municipal committee.

Your name could only be changed in a Birth Certificate for a specific period of time. This time frame is different in every state. You may contact your local municipal office for the same. Once that is changed, then your verification of documents will go through smoothly. Filing a petition for something that is just an administrative formality will be a waste of time, money and efforts.

you will have to take out a publication in a local newspaper stating this issue and announcing the new name proposed in the birth certificate. then take this publication to the concerned authority, show it to them, and ask them to cancel the previous birth certificate and issue a new one.
