Look if you marriage through some sdm office which is called Court marriage then recal l the office and get copy of your certificate.

First of all please make sure as to if a valid marriage even subsists between you and your husband or not. Secondly even if your husband has not granted you divorce and you both are living separately it means that there does exist marriage between both of you as in the absence of marriage there can be no divorce. In this case as well since your marriage has not come to end you are a married person. Therefore you should mention fact of you being married in the form but after making sure that you are lawfully married.

In this regard, it is relevant to mention that if you are sure that you performed marriage then you are obliged to disclose your status to be as married otherwise it will mis-representation on your part and can be easily used against you in court of law.
