My wife name is Laila banu but she want to change her name as Anusuya. The reason for hiding religion and numerology. What is the procedure. Pls advice me how to do!

The person has to apply to government gazattee. There is no religious change. The questions is improper. Name change should not loose any identity of the life of the person and should not violate constituency validity.
