Can a witness be charged for giving false evidence

I am a doctor. Examined a case of injury 7 year back, for which called from court for witness. That time I issued 2 written documents. One to patient , describing only the injury and treatment given. Other to officer in charge of local police station, mentioning the details, like who caused the injury, how injury happened etc. But in court i was given only the papers which I issued to the patient. The lawyer asked me about the details of injury, like how it happens, who caused it etc. But as these were not mentioned in that letter, so I replied patient did not told me about all these things. Then the lawyer showed me my second documents, where all these are clearly written. But as it was a Xerox copy and not given to me by judge , so I denied all these documents. Whether I will be charged for giving false evidence.
