Ex husband's surname was added to the name of a lady on marriage only on the marriage certificate. However no affidavit or name change in gazette or in any official government id proof was made. The marriage soon ended in divorce. But as the divorce certificate has his surname along with the lady's maiden name.. is it required to go through the process of dropping his surname? She has to fill attestation form for a job.. where It has to be mentioned whether any thing was ever added or dropped in candidate's name or surname

From the perusal of the query I would advise you as follows: (1) Yes, after a decree of divorce you are required to go the through the process of name change by executing an affidavit, public notice in newspaper and Gazette notification
