Dear querist,
From the link provided by you in the query, I understand that the Builder has provided a clause whereby the 'Declaration of Intent' for possession or refund cannot be be edited or changed, once submitted. Hence, once you, as the buyer, has opted for "possession" you cannot seek refund and vice versa.
Answering to your query in particular, if you opt for "possession" you will receive possession of the flat, only. While, If you opt for "refund", you shall get refund once you raise such demand after the stipulated date provided in the agreement. You would not get penalty and/or interest along-with principle amount paid if you opt for "possession", unless if the builder flouts the undertaking on its part. Hence, I advice you to choose wisely.
Note- I did not find the option of "Flat No." under Declaration of intent, in the home page as stated by you in the query.
