Name change in ownership documents

One plot is in the name of my father. His name in the land document is registered as M C Joseph whereas his correct and full name as per Aadhaar Card is Joseph Chandy Manayani. The second plot is registered to my mother. Her name in the land document is registered as Thankamma Joseph whereas her correct name as per Aadhaar Card is Thankamma Joseph Manayani. How can we change the names to the correct names as per Aadhaar?
Prepare a notorized affidavit stating that both names belong to the same person wirh other credentials. Now you may contact your sub registrar office. If they ask for newspaper publication and gazate notification you may do it.
