You need to file a rectification deed. Stating out the events and attach valid identity document and earlier deed. Stamp rates vary across states. This should. Rectification deed needs to be registered and executed in same manner as the earlier deed

All the parties to the agreement shall enter into correction deed and correct the name by adding name after marriage. You have to affix valid proof of marriage viz. marriage certificate, copy of name change notice published in the gazette and also the Identity proof showing name you fathers name viz. PAN, Passport etc. You have to execute and register the correction deed with concerned registrar office.

Hello,
If there is only one change than you should make addendum and if there are many changes than I suggest you to make supplementary agreement and also registered the same so you don't face any legal consequences in future.
