Advice on life insurance policy information disclosure

I purchased a Life Insurance policy in 2021 but unintentionally failed to disclose that I had undergone a piles operation in 2015. What are the chances of claim rejection in this case? Does this fall under Section 45 of the Insurance Act? Or should I consider canceling and taking a new policy?
Additionally, I smoked for a few months during late 2023 and the first quarter of 2024. Do I need to disclose this information in my existing policies?
