Adoption Deed need not to be a registered document. Neither in Hindu Adoption and Maintenance Act nor in Registration Act, it is nowhere required that Adoption Deed has to be a registered document. Even adoption can be oral if it fulfills all the requsite conditions. Therefore, you need not to get it register. If you still feel that you must get it register then biological father has to appear either in person or through his attorney.

present of parents depends upon the facts that who is taking adoption of whom (Girl or boy) but if there is no objection obtained from biological father then only mother can give or take in adoption but in some case parents who are taking adoption is mandatory required to be present before the officials.....

Yes sir biological father and adoptive father, both are to be present at the time of registration of adoption deed.
Following are the steps involved:-
1.The adoption deed is drafted/prepared by a legal expert/advocate.
2. Stamp paper is purchased fort he execution.
3.Date is fixed for the registration into sub-registrar office.
4. All parties alongwith 2 witnesses visit the sub-registrar office.
5. The adoption deed is collected after a week.
6. The ceremony of giving and taking of child is essential for making an adoption deed valid.
