Want to apply for passport with changed name what documents needed

I am going to give an exam, I have legally changed my name. Its published in the Gazzette Of India. I need to change it in my passport now. I want to apply a fresh passport in Tatkal, need to know what documents to give as in an Affidavit for the same.
As per the rules, Incase of Any change of name in Passport, Only Deed Poll Affidavit is not enough, Applicant should get a Deed Poll Affidavit & also publish advertisements in two reputed newspapers (one local newspaper & one national newspaper ) and submit original newspapers at the time of applying for passport in his/her new name.
