Surname change after marriage want to use parental surname in exam

I was known as XX as per my Matriculation certificates and even in my Masters then I got married in 2008 and my surname changed from X to Y .I continued studying after that and I did MPhil and am doing my Phd in the name of XY. I have made AAdhaar Card, Pan Card, Voters card in the name of XY. I am teaching in a reputed Private school as XY and my Bank account also stands in the same name. Now I am interested to fill UGC- NET form online which says that I must write the same name as it was there in matriculation. I also made an affidavit for change of my name from XX to XY. Now, if I pass that exam and in the future if I get selected for lectureship, which name will be mine? I want to fill up the form with XX. What to do? Should I mention XY? Do I need additional documents for change of name, or is everything ok?

In case of change of name.. both names exists... ideally in higher studies,... the names which appear in the lower school / college documents continues... you can keep writing both names with @. It is advisable to contact the authorities first and try to resolve the problems
