Procedure to change name in documents

I want to add my Surname in all my IDs & educational certificates, pls tell me full description how can proceed?

You need to get your name changed first for which you need to get a affidavit made on Rs.10/- stamp paper duly attested stating particulars as to your name, fathers name, address, current name and proposed changed name. Thereafter you need to get the same published in a newspaper. Once done you have to give an application for publication of same in gazette and attach copy of the newspaper and affidavit with it. It takes around 45 days for publication in the gazette.
Once it is published in Gazette your name is officially changed and based on same you can apply for change of name in your IDs, but your educational certificates may not be changed since they were made prior to name change. However, you can give a copy of gazette alongwith your certificates to show that the name has been changed which is totally valid.
